  • We identify functions and associated performance levels.
  • In addition to the standard 'six big losses', we analyze for management and authority induced losses and sources of  waste.
  • We analyse at root cause level.
  • If you don't get any warning, and you should, modify the design to include a sensor, gauge, alarm, etc.

  • assess risk, by multiplying values for severity, likelihood and detectability.
